For new patients, please bring:
• your insurance cards and pharmacy insurance card, if you have one.
• An updated medication and vitamin list with dosages and how often you take them.
• A copy of recent bloodwork that you have had with another physician, if you had one.
• Reports from recent x-rays, MRI’s, CT scans or Bone Density reports that are relevant to your visit with Dr. Antoniadis.
Please contact your other physicians and have them fax these documents to us at 732-359-7233. This process saves us time and allows Dr. Antoniadis to review the reports before your visit. Please arrive at least 30 minutes before your scheduled appointment to fill out our new patient forms, or if you would like, print out a copy HERE, fill them out at home and arrive at least 15 minutes before your scheduled appointment.
For a new patient appointment, you can expect to be with the doctor at least 1 hour, so please plan accordingly.
For established patients, please bring:

Do you offer X-rays, MRI’s, CT scans or physical therapy in your office?
No, we do not. We will refer you to a facility, if needed.
Do you do lab work in your office? What laboratory do you use?
Yes, we offer laboratory services for our patients. We utilize Shiel Medical laboratory as our primary lab, or if needed, per your insurance, we do have accounts with Labcorp and Quest laboratories.
